Principal Care Management (PCM)

Principal care management is a specific care management service that provides coverage for patients with two or more chronic conditions for a continuous relationship with their care team.

Under PCM, the patient’s care team can bill for time spent managing the patients' conditions. This includes:

• Formulating a comprehensive care plan
• Interactive remote communication and management (usually over the phone calls or video calls)
• Medication management
• Pulse Oximeter
• Coordination of care

There are 2 CPT codes that can be furnished by the clinical staff under the supervision of the providers (CPT 99426 and 99427) that can help you generate revenue of up to $120 per patient per month.
